In order to qualify for this offer, you must:

Accept a brief phone call (approximately 1 minute) from an Alchemy Technology Resource representative to verify your contact information so that we may process your gift card.
Submit only one request for the offer per person, per promotion.
Be currently employed by the company receiving the offer.
$50 Amazon Gift Card offer limited to the first 100 qualified survey respondents
Be an employee of an organization with a minimum revenue of $250M.
Be a decision-maker or strong influencer for IT purchases at your company (title of IT Director or higher)
Provide a business email address. Personal email addresses (addresses with gmail.com, yahoo.com, etc. will disqualify a submission)
Be a current resident of the United States.
Not be an employee or reseller of an IT technology product, solution or service.
Eligibility limited to one survey from Alchemy Technology Resource per every 60 days.
This offer is valid only for fully qualified and verifiable business contacts at companies on our email and direct mail lists. If you are not a qualified contact, or if you received this offer indirectly via a third party or some other means, or if you elected to submit your information on our website(s) without a specific invitation from us, you may not eligible for this offer.
The $50 Amazon gift card upgrade option (to bring the total incentive to $150) is for scheduling a 30-minute phone call with a representative from the IT brand sponsor of the online survey.
Limit one card per company

The promotional incentive for eligible responders will be processed within 90 days of completion of a validation conversation with one of our fulfillment associates. If an appointment was scheduled, the promotional incentive will be processed and emailed to the responder typically within one (1) day of the completion of the scheduled appointment.
Your eligibility for the $50 Amazon gift card for submitting the online survey will be determined during a telephone conversation with you following your submittal of the survey form. We reserve the right to decline processing of the offer if your actual title or employer name does not match the information you provided in the survey.
You have 30 days from the date of submitting your survey responses, to claim the $50 Amazon gift card, via an approximately 1-minute phone verification process.
